Thyroid stimulating hormone, known as TSH, is produced by the pituitary gland and serves as the primary signal that tells your thyroid gland how much thyroid hormone to make. It is often the first marker checked when thyroid health is being assessed, because it is an exquisitely sensitive indicator of whether your thyroid is producing the right amount of hormones. A TSH level within the expected range usually means the communication between your pituitary gland and your thyroid is working normally. Think of TSH as a thermostat in your home. When the room is too cold, meaning thyroid hormone levels are low, the thermostat turns the heating up by releasing more TSH, prompting your thyroid to produce more hormone. When the room is warm enough, the thermostat dials back. This feedback loop means that TSH often moves in the opposite direction to your thyroid hormones: if your thyroid is underperforming, TSH tends to rise as your pituitary gland tries to coax it into action, and if your thyroid is overproducing, TSH drops because the pituitary senses there is already plenty of hormone circulating. This is why TSH is such a powerful screening tool: even subtle shifts in thyroid function can show up as changes in TSH before the thyroid hormones themselves move outside the normal range. Various factors can affect your TSH reading. It naturally fluctuates throughout the day, tending to be highest in the early morning and lowest in the afternoon, so the time of your blood test can influence the result. Stress, illness, significant changes in weight, and certain medications, including biotin supplements, steroids, and some heart medications, can also shift your TSH. Age is another consideration, as TSH reference ranges can differ slightly for older adults. If your TSH level is outside the expected range, your GP will typically look at your free T4, and sometimes free T3, alongside it to build a clearer picture of your thyroid health and discuss any next steps with you.

Free thyroxine, or free T4, measures the unbound fraction of thyroxine circulating in your blood, the portion that is not attached to transport proteins and is therefore available for your body to convert into the active thyroid hormone T3. Because it is less affected by fluctuations in binding protein levels, free T4 is widely regarded as one of the most reliable markers of thyroid function and is routinely measured alongside TSH to give an accurate picture of how your thyroid is performing. You can think of free T4 as the readily accessible supply in your body's thyroid hormone system. While total T4 includes hormone that is bound up and essentially on hold, free T4 represents the fraction that can be immediately taken up by your tissues and converted into T3 wherever it is needed, whether that is your liver processing energy, your brain maintaining concentration, or your muscles supporting physical activity. This makes free T4 a particularly practical measurement, as it reflects the hormone that is genuinely available to do its job rather than the total amount in circulation. When your thyroid is working well, free T4 levels remain stable and your tissues receive a consistent supply of raw material for T3 production. Your free T4 levels can be influenced by a variety of factors. Changes in thyroid function are the most direct cause of shifts in free T4, but other elements play a part too. Certain medications, including levothyroxine, amiodarone, heparin, and some anti-seizure drugs, can affect your results. Severe illness, sometimes referred to as non-thyroidal illness, can temporarily lower free T4 as the body adjusts its metabolic rate during recovery. Biotin supplements, which are popular for hair and nail health, can interfere with some laboratory assays and produce misleading results, so it is worth mentioning any supplements you take when discussing your results. Pregnancy also shifts thyroid hormone dynamics, with free T4 requirements increasing during the first trimester. If your free T4 level is higher or lower than expected, your GP will interpret it alongside your TSH and clinical symptoms to understand whether your thyroid needs any additional support or monitoring.

Free T3 measures the small fraction of triiodothyronine that circulates in your bloodstream without being attached to carrier proteins. Because only the unbound, free form of T3 can enter your cells and exert its effects, free T3 is considered a particularly useful marker for understanding how much active thyroid hormone is actually available to your body at any given moment. It offers a more direct reflection of thyroid hormone activity than total T3, especially in situations where protein levels may be affecting the total measurement. Think of free T3 as the portion of your thyroid hormone that has its sleeves rolled up and is ready to work. While the vast majority of T3 in your blood is bound to proteins, essentially in transit or storage, it is the free fraction that docks with receptors inside your cells and drives the metabolic processes that keep you feeling energised, warm, and mentally focused. Free T3 influences how quickly your cells produce energy, how efficiently your heart beats, and how well your brain functions. It is the most potent thyroid hormone in its active form. Your body produces free T3 primarily by converting free T4 using deiodinase enzymes in peripheral tissues, with only a small proportion coming directly from the thyroid gland itself. Several factors can influence your free T3 levels. The conversion of T4 to T3 can be impaired by illness, stress, very low calorie diets, and deficiencies in nutrients like selenium and zinc, all of which may lead to lower free T3 even when T4 levels appear adequate. Certain medications, including beta-blockers and amiodarone, can also affect conversion. Age plays a role, with free T3 tending to decrease gradually over the decades. Because free T3 is not affected by changes in binding proteins in the same way that total T3 is, it can be especially informative during pregnancy or when taking oestrogen-containing medications. If your free T3 is outside the expected range, your GP can assess it alongside your TSH and free T4 to determine whether any further investigation or support is needed.

Total thyroxine (T4) measures the full amount of thyroxine in your blood, including both the portion bound to carrier proteins and the much smaller unbound (free) fraction. Because the vast majority of T4 is bound to proteins like thyroxine-binding globulin, the total T4 reading is heavily influenced by protein levels. Changes in binding protein (from pregnancy, oestrogen, liver disease, severe illness, or certain medications) can shift the total T4 without reflecting a real change in thyroid function. For that reason, most UK laboratories prefer free T4 (FT4) as the primary thyroid monitoring test alongside TSH. Total T4 is still useful in specific clinical contexts, such as interpreting results where binding-protein abnormalities are suspected. Think of total T4 as the total hormone in circulation, including what is already tied up in transit, whereas free T4 is the active fraction ready to be taken up by tissues. Your total T4 level is influenced by the health of your thyroid gland, your binding-protein levels, and the medications you take. Oestrogens (HRT, the combined oral contraceptive, pregnancy) typically raise total T4 by increasing thyroxine-binding globulin. Corticosteroids, androgens, and severe illness can lower it. If your total T4 falls outside the expected range, your GP will usually interpret it alongside TSH and free T4 to decide whether any further assessment is warranted.

Total triiodothyronine (T3) measures the full amount of T3 in your blood, including both protein-bound and free hormone. T3 is the biologically active thyroid hormone and is far more potent than T4 at the receptor level. Most of the T3 in your body is produced outside the thyroid gland itself, by the conversion of T4 into T3 in peripheral tissues (notably the liver, kidneys, and skeletal muscle). Because only the unbound (free) T3 fraction can enter your cells and drive metabolic activity, and because total T3 is strongly affected by binding-protein levels, free T3 (FT3) is usually the preferred test for assessing thyroid hormone availability. Total T3 still has a role, particularly when investigating T3 thyrotoxicosis (a form of hyperthyroidism where T3 is disproportionately elevated), monitoring response to T3-containing thyroid medications, or interpreting results where binding-protein abnormalities are suspected. Think of total T3 as the cargo manifest for the active thyroid hormone: it counts every T3 molecule in circulation, whether in transit or ready to deliver, while free T3 counts only the molecules currently available to work. Your total T3 level responds to the same binding-protein influences as total T4: pregnancy, HRT, and the contraceptive pill tend to raise it; androgens, severe illness, and nephrotic syndrome tend to lower it. Conversion of T4 to T3 also matters: illness, stress, very low calorie diets, and deficiencies in selenium and zinc can reduce T3 even when T4 is adequate. If your total T3 is outside the expected range, your GP will interpret it alongside TSH, free T4, and free T3 to understand what is going on.

Thyroglobulin Antibodies (TG antibodies) are proteins produced by your immune system that target thyroglobulin, a large protein made by the thyroid gland that serves as the essential scaffold for producing thyroid hormones. Your thyroid uses thyroglobulin as a kind of factory template: iodine atoms are attached to it, and the finished thyroid hormones T3 and T4 are then snipped off and released into your bloodstream to regulate your metabolism, energy, and body temperature. When your immune system produces antibodies against this protein, it can be a sign of an autoimmune process affecting the thyroid. Measuring TG antibodies is important because they offer a window into the immune system's relationship with your thyroid that you cannot get from standard thyroid function tests alone. While TSH and free hormone levels tell you how the thyroid is performing at this moment, TG antibodies, much like TPO antibodies, can reveal whether an underlying autoimmune process may be quietly influencing the gland. The two antibody tests are often requested together because they provide complementary information: some people have elevated TPO antibodies but normal TG antibodies, and vice versa, so testing both gives a fuller picture. Think of your thyroid as a small workshop and thyroglobulin as the raw material on the workbench. TG antibodies are like quality inspectors who have become overly suspicious and keep flagging the raw material as faulty, even when it is perfectly fine, which can disrupt the smooth running of the workshop over time. That said, many people with mildly elevated TG antibodies experience no symptoms and maintain entirely normal thyroid function, so an elevated result is a prompt for monitoring rather than immediate concern. Your TG antibody level can be influenced by genetic factors, particularly a family history of thyroid conditions. They are more common in women and tend to increase with age. Pregnancy, high iodine intake, smoking, and other autoimmune conditions can also play a role. If your TG antibodies are elevated, your GP can put the result in context alongside your other thyroid markers and advise on whether periodic monitoring of your thyroid function would be a sensible next step for you.

Thyroid Peroxidase Antibodies (TPO antibodies) are proteins produced by your immune system that are directed against thyroid peroxidase, an enzyme that is essential to the production of thyroid hormones. Your thyroid is a small, butterfly-shaped gland at the front of your neck, and it relies on TPO to help convert iodine from your diet into the hormones T3 and T4, which regulate your metabolism, energy levels, body temperature, and much more. When TPO antibodies are present at elevated levels, it suggests that the immune system may be interacting with the thyroid gland in a way that could affect its function over time. Understanding your TPO antibody level is important because it provides information that standard thyroid function tests (TSH, free T4 and free T3) cannot offer on their own. While those tests tell you how the thyroid is performing right now, TPO antibodies can indicate whether an autoimmune process may be at work behind the scenes, sometimes years before any change in thyroid hormone levels becomes apparent. Think of it as the difference between checking whether your car is running smoothly today versus looking under the bonnet to see if anything is slowly wearing down. The presence of TPO antibodies does not automatically mean your thyroid will stop working properly (many people with mildly elevated levels maintain normal thyroid function for their entire lives), but it does flag that closer monitoring may be worthwhile. TPO antibodies are more common in women than men and tend to become more prevalent with age. Genetic predisposition is a significant factor: if close family members have thyroid conditions, you may be more likely to have elevated levels. Other influences include smoking, high iodine intake, certain medications, stress, and hormonal changes such as pregnancy or the menopause. Selenium and vitamin D status have also been studied in relation to thyroid antibody levels, though evidence is still developing. If your TPO antibody level is elevated, your GP can advise on an appropriate monitoring schedule for your thyroid function and discuss any steps that might help support your thyroid health over the long term.
